
One of the biggest advantages of any lifting device is the ability to deliver higher energy while keeping discomfort low.
Interestingly, despite having a major celebrity ambassador, I don't think the manufacturer of CoolSonic has done a great job communicating what makes this device truly special.
And what I'm about to discuss is probably not the kind of story most marketing agencies like to tell.
Seeing Ultrasound Is Not the Same as Watching Every Shot
One of the most well-known advantages of Ultherapy is its real-time ultrasound imaging.
And to be clear, I believe this is a genuine advantage.
I use Ultherapy in my clinic, and I consider it an excellent device.
However, there is something patients should understand:
Being able to see the ultrasound image and actually checking every single shot throughout the entire procedure are two different things.
Some people misunderstand this statement and think I'm criticizing Ultherapy.
I'm not.
I'm simply describing the reality of clinical practice.
Let's take sedated Ultherapy as an example.
Patients are comfortable and relaxed, which certainly has benefits.
However, when a patient is asleep, movement can occur. Position changes can happen. Sometimes the physician's attention must shift toward monitoring vital signs and patient safety.
And when that happens, what becomes the highest priority?
Patient safety.
Always.
For that reason, the idea that every single shot is perfectly monitored in real time throughout an entire treatment session is often more complicated than patients imagine.
This isn't unique to any particular physician.
It's simply the reality of medicine.
Which is why I often tell patients that the name of the device is important, but the person using it may be even more important.
The HIFU Device I've Been Using More Frequently
Recently, one HIFU device has been getting more of my attention.
That device is CoolSonic.
The company describes it as a "cool HIFU."
From a marketing perspective, I think it's a clever phrase.
From a medical perspective, I would describe it differently:
A HIFU system equipped with active cooling technology.
At first, I assumed the cooling feature was mostly about comfort.
After using it extensively, I realized it offers much more than that.
The Biggest Advantage Is Not Less Pain
Many clinics advertise CoolSonic by saying:
"It doesn't hurt."
And honestly, that's not entirely wrong.
My own father has undergone treatment with it.
Several friends and acquaintances who are particularly sensitive to pain have received it as well.
To date, nobody has ever complained to me afterward about severe discomfort.
Many patients tolerate treatment without topical anesthesia, and most patients are quite comfortable when numbing cream is applied.
For people considering sedation simply because they're worried about pain, CoolSonic may be worth considering.
But in my opinion, the real advantage is something else.
Lower discomfort allows physicians to use higher energy settings more confidently.
And that can matter.
No matter how advanced a device is, if the treatment becomes too painful, both the physician and the patient naturally become more conservative.
CoolSonic changes that equation.
Cooling Technology Does More Than Improve Comfort
Many patients are unaware of this.
During treatment, CoolSonic continuously cools the skin surface while creating thermal coagulation points at the target depth.
In other words:
- Heat is being delivered where treatment is needed.
- Cooling is simultaneously protecting the surface.
As a result, patients often experience not only reduced discomfort but also less swelling.
Some patients even describe the experience as feeling similar to having received a calming post-treatment facial afterward.
I've noticed the same thing in practice.

- Under-eye areas

And in medicine, having more options is often valuable.
What About Facial Volume Loss?
Many patients searching for lifting treatments are concerned about facial hollowing.
My answer is simple:
No lifting treatment is 100% risk-free.
You'll occasionally see advertisements claiming:
- Thermage never causes hollowing.
- Sofwave never causes volume loss.
Personally, I don't agree with those absolute statements.
In reality, volume loss can occur with many energy-based devices if heat affects fat tissue.
The real question isn't the name of the device.
It's:
- How deep is the energy delivered?
- How much energy is used?
- Which tissues are being targeted?
That's where treatment planning becomes more important than marketing.
Some Patients Want Wrinkle Improvement Without Botox
I remain a big fan of skin Botox for fine wrinkles.
The results are excellent.
However, not everyone wants Botox.
Some people simply prefer to avoid any possibility of altering facial expressions.
This includes:
- Flight attendants
- Actors
- Broadcasters
- Streamers
- Sales professionals
- Public speakers
For these patients, a superficial 1.5 mm HIFU approach can be an interesting alternative.
It allows us to target:
- Fine lines
- Skin texture
- Skin tightening
without affecting facial movement.
The Most Important Thing Is Having Choices
One of the most common questions I hear is:
"Doctor, which is better? Ultherapy or CoolSonic?"
The truth is that the question itself is often too simple.
Different faces have different needs.
Some patients have abundant facial fat.
Others have very little.
Some are concerned about jawline definition.
Others are worried about under-eye laxity.
There is no universal answer.
That is why I prefer not to recommend devices blindly.
Why Our Clinic Invests in So Many Devices
This is actually the philosophy behind our clinic.
If a clinic owns only one lifting device, even the most honest physician will naturally explain treatment through the lens of that device.
Because there are no alternatives.

However, if the only takeaway from this article is "I should get CoolSonic," then I haven't accomplished my goal.
Instead, I hope readers leave with a different realization:
- Facial lifting is more complex than most advertisements suggest.
- Diagnosis matters more than device names.
- The physician matters as much as the technology.
