EXION RF Microneedling Review: A Doctor’s First-Hand Experience After a Live Demo
First Impressions of the Device
The EXION RF Microneedling system by BTL is a premium device with a price tag exceeding 100 million KRW (approximately $70,000 USD).
Interestingly, its external design is quite similar to another BTL device that we currently own, called Vanquish, which is used for body contouring. Looking at the carts and overall design, I couldn't help but wonder whether BTL is making efficient use of the same manufacturing platform across multiple devices. 😊

Whenever a new aesthetic device enters the market, I prefer to see it in person, handle it myself, and whenever possible, perform treatments on my staff before considering it for patients.
As physicians, we hear countless claims from manufacturers. The only way to truly understand a device is to use it ourselves and compare those claims with real-world clinical experience.
Recently, I had the opportunity to perform a live demo treatment using the EXION RF Microneedling system.
What Initially Made Me Curious About EXION?
To be honest, EXION was not originally on my radar.
My interest actually began years ago when I read about the Profound RF system by Candela.
Profound has a reputation for delivering impressive results, but it is also known for being extremely painful. Many providers perform it under sedation because topical anesthesia alone is often insufficient.
After watching treatment videos and reading patient experiences, one question kept coming to mind:
"If these RF microneedling devices cost over six figures, what exactly makes them different?"
That curiosity stayed with me.
Then I heard an interesting story.
One of our staff members has a friend who works at another clinic. Their clinic had recently hosted an EXION demo, and the feedback was unexpected.
It wasn't about lifting.
It wasn't about tightening.
Instead, the comments were:
"My nose became incredibly smooth."
"My blackheads seem noticeably reduced."
"My pores look tighter."
Those are not the typical comments people make after a lifting treatment.
Naturally, that made me curious.
So I decided to evaluate the device myself.
The Demo Treatment
The treatment was performed on a female staff member in her late 30s.
Areas treated included:
- Full face
- Double chin
- Neck
- Upper eyelids
- Lips
Originally, we planned to begin immediately after topical anesthesia.
However, our schedule unexpectedly became delayed.
The numbing cream was removed, and approximately 20–30 minutes passed before treatment began.
At that moment, I became slightly concerned.
RF microneedling treatments involve needles penetrating the skin, and inadequate anesthesia can significantly affect patient comfort.
Treating the Eyelids
One area that interested me was the upper eyelid.
By gently lifting the eyelid skin upward and treating the tissue over the orbital rim, it is possible to cover approximately 80–90% of the upper eyelid area.
Our clinic currently uses Virtue RF, which is one of my preferred devices for eyelid rejuvenation and skin remodeling.
EXION also impressed me in this regard.
Its handpiece has a very compact treatment tip, making it easy to work in small and anatomically delicate areas such as the eyelids.
There are relatively few devices suitable for eyelid treatment, so both EXION and Virtue RF possess an important advantage in this category.
Virtue RF still has one notable benefit: its surface cooling system significantly reduces treatment discomfort.
From a patient's perspective, that is a meaningful advantage.
Treating the Lips
Another interesting aspect was lip treatment.
Throughout my career, I have used numerous RF microneedling systems, including Potenza, Virtue RF, Secret RF, Scarlet RF, Agnes RF, and several others.
Despite that experience, I rarely treated the lips.
The reason is simple:
Pain and bleeding.
The lips are highly vascular, making treatment challenging.
However, after observing how little bleeding occurred with EXION, I decided to treat the lips as well.
To my surprise, bleeding was virtually nonexistent.
Not just minimal—almost none.
That alone represents a significant advantage.
Recently, procedures such as "Lip Rejuran" have become increasingly popular in Korea, involving injections of Rejuran Healer and skin boosters into the lips.
While injectable treatments can certainly improve hydration and appearance, RF microneedling may offer a more fundamental approach by stimulating tissue remodeling directly.
Of course, using an entire disposable tip solely for lip treatment would be inefficient.
In practice, lip treatment would likely be performed alongside a full-face session.
For reference, a single EXION tip is programmed for 1,000 shots.
Surprisingly Comfortable
The biggest surprise was the level of discomfort.
According to the staff member who received treatment:
- Ultherapy: 8/10
- Thermage: 6/10
- EXION RF Microneedling: 3/10
Naturally, discomfort was somewhat greater around bony areas such as the forehead and periocular region.
There were moments when tears nearly appeared.
However, overall, the procedure remained very tolerable.
If I were to compare it with other RF microneedling devices, I would say it felt only slightly more uncomfortable than Virtue RF, which is already known for relatively low treatment pain.
Considering that anesthesia was less than ideal, I suspect patient satisfaction would be quite good under normal circumstances.
Why Was It Less Painful?
I cannot say with certainty.
However, one factor that immediately stood out was needle quality.
EXION utilizes 31-gauge needles.
On paper, that specification may not seem revolutionary.
In practice, however, the needles felt exceptionally sharp and refined.
According to the company representative, significant attention was paid to both needle manufacturing and gold coating quality.
These details are difficult to appreciate from a brochure.
But during actual treatment, they become noticeable.
The sensation of skin penetration, the smooth entry and exit of the needles, and overall consistency all contribute to the treatment experience.
Fast Treatment Speed Matters
I also believe treatment speed plays a role.
Some older RF microneedling systems operate relatively slowly.
Patients feel every insertion and withdrawal, which can increase anticipation and discomfort.
EXION is remarkably fast.
The needles enter quickly and exit quickly.
Yet despite that speed, the movement feels controlled and stable rather than abrupt.
From an operator's perspective, it was a very comfortable device to use.
Extremely Minimal Bleeding
This was probably the most impressive aspect.
With many RF microneedling devices, including Potenza, some degree of pinpoint bleeding is expected.
During this full-face treatment, however, bleeding was minimal enough that a single alcohol pad was sufficient for cleanup.
That level of hemostasis was genuinely impressive.
Of course, individual patients will vary.
But this particular treatment left a strong impression.
One Drawback: Noticeable Post-Treatment Swelling
No device is perfect.
Although bleeding was minimal, post-treatment redness and swelling were more noticeable than I expected.
Specifically, there was visible "embossing" or raised swelling corresponding to the treatment pattern.
As I write this article, the treatment was performed only recently.
The face remains somewhat swollen.
Therefore, it is far too early to evaluate the final outcome.
Compared with Potenza, EXION seemed to produce less bleeding but slightly more visible immediate downtime.
Patients with important social events should be informed accordingly.
My expectation is that the embossing effect will improve within one to two days.
The AI Impedance Monitoring System
Manufacturers often attach the term "AI" to almost everything these days.
As a result, I approached this feature with some skepticism.
However, EXION's impedance monitoring system was genuinely interesting.
Each needle within the 36-pin tip continuously measures impedance and adjusts energy delivery accordingly.
When contact is optimal, a blue light appears.
When contact is inadequate, a red light appears.
This became particularly useful in softer treatment areas such as the neck.
By stretching the skin appropriately, consistent contact could be achieved, producing immediate visual feedback.
It is reassuring to know exactly when tissue contact is optimal.
A Surprisingly Useful Cleaning Mode
This is not a glamorous feature.
However, it may be one of the most practical.
Patients with oily skin often leave sebum and debris between needle arrays during treatment.
EXION includes a cleaning mode designed to help maintain tip performance during procedures.
Manufacturers frequently focus on headline features.
Yet small details like this often make the biggest difference during everyday clinical use.
From the Physician's Perspective
One aspect I genuinely appreciated was the handpiece weight.
It is surprisingly light.
Patients rarely think about this.
Physicians do.
After treating multiple patients throughout the day, wrist fatigue becomes a real consideration.
A lightweight handpiece improves comfort significantly.
The main downside was the absence of a foot pedal.
Instead, the operator continuously presses the handpiece button.
Imagine clicking a computer mouse hundreds or even thousands of times.
Some practitioners may dislike this design.
Personally, I did not find it particularly problematic.
So... Does It Actually Work?
The honest answer is:
I don't know yet.
The treatment was performed today.
Swelling and embossing are still present.
It is simply too early to evaluate lifting, tightening, or collagen remodeling outcomes.
Interestingly, the result I am most curious about isn't lifting at all.
It is the same question that sparked my interest in the first place.
Can it really make the nose feel smoother?
Can it genuinely improve blackheads and enlarged pores?
RF microneedling is already known to improve skin texture and pore appearance.
The real question is how much improvement EXION can deliver in actual practice.
If my staff member comes back in a few days and says,
"Doctor, my nose is unbelievably smooth."
then I may have another follow-up article to write.
Final Thoughts
From what I understand, EXION is still relatively uncommon in Korea.
It is also an undeniably expensive device.
After this hands-on experience, my impression is not that EXION is a revolutionary machine that changes everything.
Rather, it feels like a device that has refined the fundamentals exceptionally well.
- High-quality needles
- Minimal bleeding
- Fast treatment speed
- Real-time impedance monitoring
- Lightweight handpiece
- Thoughtful clinical details
Each improvement may seem small on its own.
Together, they create a highly polished treatment experience.
Whenever I evaluate a new device, I do not ask:
"Is this the one device that does everything?"
Because no such device exists.
Instead, I ask:
"What does this device do particularly well?"
"Which patients might benefit most from it?"
That is why I continue to attend demonstrations, test devices personally, and perform treatments before making clinical decisions.
Brochures tell one story.
Real-world experience tells another.
EXION was certainly an interesting experience.
But if I'm being completely honest...
I'm still most curious about that smooth nose. 😄
